The Africa Care Digital Health ID is your personal health identity, linked to a QR code on your phone. It helps every clinician you see identify you correctly, carries the emergency information you choose to share, and gives you one place to see and control your health information.

Health ID
Sign up at id.africa-care.com, or claim the invitation your practice sends you. You hold one Digital Health ID, and it stays yours.
Your emergency profile is off by default. You switch on each field individually — allergies, conditions, medication, an emergency contact — and can preview exactly what a stranger would see.
Show your QR code at reception or keep it in your phone's wallet for emergencies. You can suspend and reissue it at any time if a card is lost.
Scanning your code never opens your health record by itself. What a scan reveals depends on who is scanning.
When your patients hold a Digital Health ID, the information you need arrives with them — instead of being rebuilt from memory at every visit.
No spelling names at the front desk, no duplicate folders. Correct identification is a patient-safety step, and the ID makes it the default.
Consultation, discharge and referral summaries associated with the ID follow the patient between the providers they use — with their permission, and with every access logged.
Details the patient keeps current — contact numbers and, if they choose, medical aid — are there when they arrive, so your team isn't re-capturing forms.
Allergies, significant conditions and critical medication the patient chose to share are one scan away — an aid to your assessment, never a replacement for it.
Set it up in a few minutes at id.africa-care.com.
The Digital Health ID supports care but never replaces professional medical judgement, and it is not an emergency service. In an emergency in South Africa, call 112 from a mobile phone or 10177 for an ambulance. The full Digital Health ID Terms and Privacy Policy are published at id.africa-care.com; questions go to patients@africa-care.com.
